How to build a form with multiple questions in Surveys & Forms
What you will achieve in this guide
Follow the feature-specific setup and testing instructions below. Check the supported pages and plan requirements before activating the feature.
Create a short post-order survey containing more than one question, then verify the answers in the selected form’s submissions view.
When to use this guide
Use this when one answer alone is not enough—for example, discovery channel plus optional customer feedback.
Before you begin
Use a test customer and order that you can identify. Confirm the feature is active, placed on a supported page, and included in your Rivio plan.
Worked example
Create a form called Discovery feedback with How did you hear about us? and Is there anything you would like us to improve? Use Instagram, Google, and A friend for the first question.
Step-by-step instructions
- Open UI extension blocks → Surveys & Forms and create or manage the form.
- Add the discovery question using an option-based input type offered by the editor. Enter each choice separately.
- Add the feedback question using a supported text input. Keep the questions short and clearly labelled.
- Review question order and the form preview; confirm each option belongs to the intended question.
- Save the active form and add its matching block separately to the supported Thank you or Order status page.
- Complete a test order, select Instagram, enter a recognisable feedback answer, and submit through the form’s action.
- Open the submissions view for that exact form and compare both answers with the submitted values.

Verify the result
| Test | Expected result or check |
|---|---|
| Both questions answered | The submitted record contains the corresponding answers. |
| Form viewed without submission | Do not assume a response record is created. |
| Phone layout | All questions and choices remain readable. |

Limitations
- Use only question types offered by the app.
- A post-order survey cannot prevent a completed order.
- Viewing a form is different from submitting it.
Troubleshooting
| Problem | What to check |
|---|---|
| The expected result does not appear | Check the saved state, exact feature configuration, supported page, and matching test inputs. |
| The preview and actual page differ | Use a real test checkout, completed order, or signed-in account; editor data is simulated. |
| A change has an unexpected effect | Repeat the same test with one relevant setting changed. Review other active rules before adding another copy. |
