How to Select the Correct Form Using Its Slug

The Form slug setting tells the Shopify Wholesale form app block which Rivio registration form to display.

Before you begin

Save and publish the form, copy its exact slug, and identify the theme and page template you will edit. Check whether other pages share that template before adding or moving its app block.

Why the slug must match

The slug identifies the form to the storefront extension. It is not the internal name or visible title. A typo, extra space, or old slug can prevent the intended form from loading or select a different form.

Give retailer and distributor forms distinct slugs. Whenever you change a slug in the app, update every corresponding theme block; renaming the admin form alone is not a reliable way to change placement.

Set up this workflow

  1. Open Forms and select the intended published form.

  2. Copy its slug from the form details.

  3. Open the theme editor and select the Wholesale form block.

  4. Paste the exact value into Form slug, avoiding extra spaces.

  5. Save the theme configuration.

  6. Visit the page and verify the questions belong to the intended form.

Example

Create forms with slugs demo-retailer and demo-distributor. Enter demo-retailer in the test page's Wholesale form block.

Expected result

The page shows the retailer questions rather than the distributor form.

Limits and important behavior

  • Changing the admin form name does not necessarily change its slug.
  • If a slug changes, update every block using the previous value.
  • Use distinct slugs for wholesale, retailer, and distributor forms.

Troubleshooting and verification

If the form does not load, compare the saved slug, publication status, page template, and theme. If a popup does not open, check its display mode and trigger; a timed or exit-intent popup does not behave like an inline form.
