How to Add the Wholesale Form Block to Your Shopify Store

What you will achieve in this guide

Follow this step-by-step documentation to configure custom B2B registration forms, approval workflows, and customer tagging.

EXPECTED SETUP TIME 5-10 Minutes
COMPATIBILITY All Shopify Themes & Plans
TECHNICAL DIFFICULTY Zero Code Required

Display a published Shopify registration form by adding the Wholesale form app block and entering its form slug in the theme editor.

Before you begin

Save and publish the form, copy its exact slug, and identify the theme and page template you will edit. Check whether other pages share that template before adding or moving its app block.

Connect a page to a saved form

The Online Store 2.0 Wholesale form app block loads a form by slug. Save and activate the form before adding the block, then enter that exact slug in the block settings. No theme-code edit is needed for this placement.

A page template can be shared by several pages. Choose the actual template assigned to the intended page and review other pages using it before adding a block that will appear on all of them.

Set up this workflow

  1. Save and publish the form in the app, then copy its slug.

  2. Open Shopify Online Store → Themes and open the theme editor.

  3. Select the page or page template where the registration form should appear.

  4. Choose Add section or Add block as supported by the location, then Apps → Wholesale form.

  5. Enter the saved Form slug and review the displayed form.

  6. Save the theme and visit the intended storefront page to confirm the form appears.

Example

Create a demo page, open its assigned page template in the theme editor, add Wholesale form, and set Form slug to demo-wholesale.

Expected result

The actual page displays the intended active form after both app and theme settings are saved.

Limits and important behavior

  • The form must be active and the slug must match exactly.
  • A page template can be shared by several pages; check which pages use it.
  • No theme-code editing is required for the Online Store 2.0 app block.

Troubleshooting and verification

If the form does not load, compare the saved slug, publication status, page template, and theme. If a popup does not open, check its display mode and trigger; a timed or exit-intent popup does not behave like an inline form.

Screenshots to add

Screenshot placeholder: Capture the relevant app screen for “How to Add the Wholesale Form Block to Your Shopify Store” after completing the setup. Show the controls used to save and publish the form in the app, then copy its slug. Add a second screenshot showing the result of this check: Save the theme and visit the intended storefront page to confirm the form appears. Use dummy applicant details and mask any secrets.
