How to Open a Form with a Popup Button

What you will achieve in this guide

Follow this step-by-step documentation to configure custom B2B registration forms, approval workflows, and customer tagging.

EXPECTED SETUP TIME 5-10 Minutes
COMPATIBILITY All Shopify Themes & Plans
TECHNICAL DIFFICULTY Zero Code Required

Use the popup-button display mode to open a Shopify registration form in a modal when a visitor selects your button.

Before you begin

Save and publish the form, copy its exact slug, and identify the theme and page template you will edit. Check whether other pages share that template before adding or moving its app block.

What the popup button controls

Popup, button keeps the form in a modal until the visitor selects the configured button. The block's button label should explain the action, such as Apply for wholesale, and its slug selects the form opened in the modal.

Popup mode changes presentation only. It does not change the application questions, notifications, or review policy. Check opening, closing, reopening, keyboard movement, and mobile completion on the storefront.

Set up this workflow

  1. Open the Wholesale form block in the Shopify theme editor.

  2. Enter the published form’s slug.

  3. Choose the popup-button display mode.

  4. Set a descriptive Popup button label, such as Apply for wholesale access.

  5. Save and view the live page.

  6. Select the button, complete the form, and verify the application arrives in Submissions.

Example

Set Popup, button and use Apply for wholesale as the trigger label. Open the popup, complete the form, and test closing it.

Expected result

The button opens the correct form and the visitor can complete or close the modal.

Limits and important behavior

  • Popup display does not change the form’s approval settings.
  • Check closing, reopening, keyboard navigation, and mobile display.
  • Use a label that accurately explains what happens when the button is selected.

Troubleshooting and verification

If the form does not load, compare the saved slug, publication status, page template, and theme. If a popup does not open, check its display mode and trigger; a timed or exit-intent popup does not behave like an inline form.

Screenshots to add

Show the controls used to open the Wholesale form block in the Shopify theme editor. Add a second screenshot showing the result of this check: Select the button, complete the form, and verify the application arrives in Submissions. Use dummy applicant details and mask any secrets.
