How to Create a VIP Customer Application Form
What you will achieve in this guide
Follow this step-by-step documentation to configure custom B2B registration forms, approval workflows, and customer tagging.
Create a custom VIP application in Shopify by adapting the Trade Professional Program template or a blank registration form.
Before you begin
Choose the template that matches your audience, then inspect its questions, approval tags, and company-creation defaults. Decide which information your reviewer actually needs before adding more required questions.
Create a custom VIP workflow
There is no separate VIP template in the documented nine-template collection. Start with Trade Professional Program or a blank form, then add the eligibility questions relevant to your VIP audience.
Use a clear approval tag such as demo-vip for testing and manually review evidence that needs human judgment. A VIP tag identifies the customer; discounts, exclusive content, or special pricing require your separate Shopify configuration.
Set up this workflow
Open Templates and choose Trade Professional Program, or create a blank form.
Name the form for your VIP program and choose a unique slug.
Collect contact details and the business information you actually use to evaluate applicants.
Review the vip approval tag and add other appropriate tags.
Configure manual review or approval rules, then save and publish when ready.
Approve a test applicant and verify the customer tags and any resulting segment membership.
Example
Use the Trade Professional template as a starting point for a designer program. Add a portfolio question and configure a distinct demo-vip approval tag.
Expected result
The custom application reflects the program without implying a separate built-in VIP template.
Limits and important behavior
- The documented template collection does not contain a separate VIP template; this guide describes a customized form.
- VIP tags do not automatically enable discounts or exclusive content.
- Do not collect purchase volume unless it is relevant to the program.
Troubleshooting and verification
If the form collects responses but does not grant wholesale access, inspect the application status and after-approval settings. A template supplies a starting configuration; pricing, catalogs, and access in your other tools still need their own setup.

Show the controls used to open Templates and choose Trade Professional Program, or create a blank form. Add a second screenshot showing the result of this check: Approve a test applicant and verify the customer tags and any resulting segment membership. Use dummy applicant details and mask any secrets.
