Choosing the Right Form Template
What you will achieve in this guide
Follow this step-by-step documentation to configure custom B2B registration forms, approval workflows, and customer tagging.
Choose a Shopify registration template for wholesale, retailer, trade, distributor, contact, feedback, quote, newsletter, or survey applications.
Before you begin
Choose the template that matches your audience, then inspect its questions, approval tags, and company-creation defaults. Decide which information your reviewer actually needs before adding more required questions.
The nine starting templates
The available starting points are Wholesale Application, Retailer / Stockist Application, Trade Professional Program, Distributor / Bulk Buyer, Contact, Feedback, Quote Request, Newsletter Signup, and Survey. Choose by the information and audience rather than by a promised outcome.
Business-onboarding templates can include approval tags and company settings. General-purpose forms do not need company creation just because this is a B2B app. Inspect the generated settings before collecting live responses.
Example
Compare Wholesale Application with Retailer / Stockist Application for a merchant recruiting stores to carry its products. Review the stockist template's questions and approval settings before choosing it.
Expected result
The selected starting point matches the audience, and template defaults have been checked before activation.
Limits and important behavior
- The documented collection contains nine templates.
- Review settings instead of assuming a template grants wholesale pricing automatically.
- Approval tags can support your other Shopify workflows, but tags alone do not assign B2B catalogs.
Troubleshooting and verification
If the form collects responses but does not grant wholesale access, inspect the application status and after-approval settings. A template supplies a starting configuration; pricing, catalogs, and access in your other tools still need their own setup.
Screenshots to add
