How to Use the Survey Template

What you will achieve in this guide

Follow this step-by-step documentation to configure custom B2B registration forms, approval workflows, and customer tagging.

EXPECTED SETUP TIME 5-10 Minutes
COMPATIBILITY All Shopify Themes & Plans
TECHNICAL DIFFICULTY Zero Code Required

Use the Survey template in Rivio B2B Registration Forms to collect Shopify applications from visitors answering a short questionnaire.

Before you begin

Choose the template that matches your audience, then inspect its questions, approval tags, and company-creation defaults. Decide which information your reviewer actually needs before adding more required questions.

Use consistent answers for comparison

A Survey form collects responses about a defined topic. Use single-choice questions for one selected answer, checkboxes for multiple interests, and a paragraph for explanations. Keep wording consistent so you can compare answers in submissions or CSV.

A survey is not a checkout or payment flow. Add contact fields only where relevant and review rules so a simple survey response does not unintentionally create wholesale access.

Set up this workflow

  1. Open the app Templates page.

  2. Find Survey and review its purpose.

  3. Select Use template to open the generated form.

  4. Review the survey questions and any contact details your use case needs; adjust required fields and validation for your workflow.

  5. Review the after-approval customer tags and company-creation option. Keep only the settings appropriate to this form and save.

  6. Preview the saved form and submit a test to confirm the collected answers support your review process.

Example

A brand asks potential wholesale buyers which product ranges interest them most.

Expected result

The chosen product interests appear in the recorded answers and can be reviewed or exported.

Limits and important behavior

  • Template fields and settings can be customized before publication.
  • Customer tags identify applicants but do not automatically grant a price list or catalog.
  • Add an Email field or Customer block if the workflow needs applicant emails or email-marketing integrations.

Troubleshooting and verification

If the form collects responses but does not grant wholesale access, inspect the application status and after-approval settings. A template supplies a starting configuration; pricing, catalogs, and access in your other tools still need their own setup.

Add a second screenshot showing the result of this check: Preview the saved form and submit a test to confirm the collected answers support your review process. Use dummy applicant details and mask any secrets.
