Creating Contact and Feedback Forms from Templates

What you will achieve in this guide

Follow this step-by-step documentation to configure custom B2B registration forms, approval workflows, and customer tagging.

EXPECTED SETUP TIME 5-10 Minutes
COMPATIBILITY All Shopify Themes & Plans
TECHNICAL DIFFICULTY Zero Code Required

Use Shopify contact forms for enquiries and feedback forms for customer experience information with Rivio B2B Registration Forms.

Before you begin

Choose the template that matches your audience, then inspect its questions, approval tags, and company-creation defaults. Decide which information your reviewer actually needs before adding more required questions.

Choose based on the response you need

Use Contact when the visitor is requesting a reply to an enquiry. Use Feedback when the visitor is describing an experience or evaluating something. The first usually needs a reachable contact; the second may only need the answers relevant to your review.

Both create submissions that your team can inspect. Set notification recipients and confirmation wording for the purpose of the form, and avoid enabling B2B company creation for general messages.

Example

Create one Contact form asking for an enquiry and a separate Feedback form asking about onboarding experience.

Expected result

A product enquiry and an experience review enter the appropriate form's submission list.

Limits and important behavior

  • These are general-purpose forms; company creation is not required simply because the app also supports wholesale applications.
  • Review data collection and marketing consent for your intended follow-up.

Troubleshooting and verification

If the form collects responses but does not grant wholesale access, inspect the application status and after-approval settings. A template supplies a starting configuration; pricing, catalogs, and access in your other tools still need their own setup.
