What you will achieve in this guide

EXPECTED SETUP TIME 5-10 Minutes
COMPATIBILITY All Shopify Themes & Plans
TECHNICAL DIFFICULTY Zero Code Required

Create a Shopify wholesale application form from a template, customize its fields, and prepare it for storefront publication.

Before you begin

Identify the form by both its internal name and slug before making changes. Check where that slug is used on your storefront, and review dependent rules or mappings before removing questions.

Decide what to collect before building

For a wholesale application, begin with a reachable email, business identity, and the information you use to qualify buyers. Add documents or annual purchasing only if your reviewer needs them. A shorter form is easier to complete and review.

The internal name helps your team identify the form; the slug connects it to the storefront block. Template tags and company-creation defaults affect approval, so review them before publishing rather than accepting every default without checking.

Set up this workflow

  1. Open the app and select Create a form.

  2. Choose a template suited to wholesale, retailer, trade, or distributor applications.

  3. Enter an internal form name and a unique URL-friendly slug.

  4. Review the template fields, remove unnecessary questions, and configure required fields.

  5. Save the form, review its appearance and approval settings, and preview it.

  6. Publish the form and confirm the saved slug is ready for the storefront block.

Example

Name the form Demo Wholesale Application and use the slug demo-wholesale. Collect business name, applicant email, country, and a resale-certificate upload. Keep decisions manual for the initial test.

Expected result

A storefront test is received under the correct form, with the answers available for review.

Limits and important behavior

  • The internal name identifies the form in admin; the slug connects the form to its theme block.
  • Review template approval tags and company settings before accepting live applications.

Troubleshooting and verification

If the preview and storefront differ, save the builder, reopen the form to confirm the saved version, and compare its slug with the Wholesale form block. A similarly named form can be a separate copy with different settings.
